Form I9 Employment Verification
What is a Form I-9?
A Form I-9 is used to verify the identity and employment eligibility of anyone hired for employment in the United States. The employee will confirm that he or she is authorized to work in the U.S. and the employer will also complete the form upon hiring an individual.
What information is requested on a Form I-9?
- Name
- Address
- Date of Birth
- US Social Security Number (if you have one)
- Citizenship Status
- Signature
- Documentation of citizenship: This may include a Passport, Resident Alien Card, Employment Authorization Document, or a combination of separate documents that establish your identity and documents that establish employment authorization.
