GENERAL FUNCTION
Provide support to a group of Blood Pressure Self-Monitoring program participants as they
adopt the habit of self-monitoring and tracking their blood pressure in order to achieve
better blood pressure management.
QUALIFICATIONS:
- Y-USA Healthy Heart Ambassador certification
- Strong Listen First skills
- Empathy toward Health Seekers with chronic diseases
- Ability to provide positive feedback and build relationships
- Working knowledge of high blood pressure and the wants, needs, and interests of
- adults with high blood pressure
- Consistently positive, patient, and non-judgmental attitude toward participants
- Ability to follow strict program delivery guidelines with consistency
- Ability to effectively utilize computer software technology to manage program
- activities and track participant progress, if assigned REDCap data entry duties
- Preferred: strong facilitation skills
PRINCIPAL DUTIES:
- Assist with program promotions and participant recruitment activities as directed by
- Program Manager
- Enroll new participants in the Blood Pressure Self-Monitoring program
- Send weekly reminder and support messages to caseload of participants
- Provide 10-minute consultations to program participants during established Office
- Hours
- Measure and record participant blood pressures, modeling correct technique, during
- Enrollment, Office Hours, and at Nutrition Education Seminars
- Utilize REDCap web-based software to track participant information, Office Hours and
- Nutrition Education Seminar attendance, and blood pressure readings in an accurate
- and timely manner as assigned by program manager
- Maintain participant confidentiality by managing program records and personal
- health information according to best practices
- Accept payment for class and provide receipts to participants when
- conducting class at a non-Y location
- Facilitate 60-minute Nutrition Education Seminars as needed
PHYSICAL REQUIREMENTS:

Specific vision abilities required by this job include close vision, distance vision, and the ability to adjust. The noise level in the work environment is usually moderate. Sufficient strength, agility and mobility to perform essential functions of position and to supervise program activities.
