The Habilitation Coordinator at the Lexington Arc works with individuals and staff to develop and write individualized Residential and Day Habilitation/Staff Action Plans based on the valued outcomes established by the individual and their advocate. The Habilitation Coordinator will attend team meetings and monitor the implementation of Habilitation/Staff Action Plans at both day program and the individuals home while ensuring consistent, quality execution of plan by providing training to staff and managers as needed.
Bachelor's degree in Human Services or related field plus one-year working experience in a setting supporting individuals with developmental disabilities, or an Associate's degree in Human Services or related field plus four years working experience in a setting supporting individuals with developmental disabilities is required. Familiarity with residential and/or day program operations is preferred. A valid NYS Driver's License required. Must have own transportation.
