Sycamore Services is seeking an Occupational Therapist

Occupational therapists lead the process in development, implementation, and coordination of the occupational therapy program.

Screening, evaluation, intervention planning, therapeutic intervention, and exit planning is provided for patients identified with or suspected of having disabilities that interfere with their ability to perform daily life activities or participate in necessary or desired socially expected functions or occupations.

Professional judgment and clinical knowledge are used to develop individualized programming based on occupational performance deficits in the areas of personal care, societal role, interaction skills, process skills, play, community integration/work, and written communication.

The Occupational therapist regularly collaborates with other disciplines and services at departmental and system levels. Occupational therapist is expected to independently review outcomes and modify intervention programs. Attends and/or conducts in-service training programs and provides consultation to agency personnel as needed.

Performs related duties as assigned, ensuring the best interests of agency and clients.

Job Qualifications

  • Have successfully completed and graduated from an accredited occupational therapy professional program, licensed and registered by the Health Professions Bureau of Indiana, and certified by the National Board for Certification of Occupational Therapy
  • Two years of experience as an Occupational Therapist preferred, preferably in school system or other pediatric practice setting
  • Must be enrolled or be able to be enrolled in the First Steps System
  • Thorough knowledge of and ability to make practical application of theories and methods in testing and evaluating fine motor skills of developmental delayed/at risk children
  • Working knowledge of Individualized Family Service Plan (IFSP) guidelines, with the ability to develop and implement appropriate goals and objectives to address individual fine motor and self help needs
  • Excellent written and oral communication skills are essential
  • Ability to follow all personnel policies and rules of the agency, and maintain confidentially of records and information as required
  • Must have effective transportation and valid driver's license
  • Ability to work non-standard flexible schedule and prioritize and schedule work tasks independently.

Essential Functions

  • Service Delivery
    • Develops occupationally based intervention plans based on patient needs and evaluation results
    • Provides targeted, evidence-based therapeutic intervention to facilitate improved function and participation within the patient's environments
    • Evaluates the patient's abilities and disabilities through a variety of functional, behavioral, and standardized assessments, skilled observation, checklists, histories, and interviews
    • Incorporate parent/caregiver involvement in order to encourage follow-through and integration of the fine motor skills in all aspects of the patient's daily life. Educates patient, educational personnel, and family to facilitate improved functional skills as well as health maintenance and safety
    • Adapts and modifies the environment including assistive technology and provides training to caregivers and community childcare staff to meet individual needs and to help patients function as independently as possible
    • Ability to work in classrooms, client homes and in a standard office environment involving pushing/pulling objects, handling/grasping, fingering objects, crawling, crouching/kneeing, reaching, bending at waist, transferring, mobility training and/or repeatedly lifting patients weighing more than 50 pounds, lifting or transferring equipment. May be occasionally exposed to unsanitary conditions and communicable diseases
    • Monitors and assess the effects of occupational therapy intervention and the need to continue, modify, or discontinue intervention
    • Help clients improve decision making, abstract reasoning, memory, sequencing, coordination and perceptual skills, including using computer programs
    • Evaluate, analyze and prepare programs for sensory modulation in children with sensory impairment
    • Evaluate, analyze including incorporating swallow study data, and prepare intervention strategies for children with feeding and swallowing disorders
  • Training and Support
    • Teaches, monitors, and collaborates with educational personnel, community agencies, parents, and patients to increase understanding of the patient's needs and performance
    • Collaborates with other disciplines to ensure team understanding of patient performance strengths and needs, through evaluation, intervention planning, and service delivery
    • Use professional literature, evidence based research, and continuing education content to make practice decisions
    • Provides continuing education and in-services for colleagues and caregivers
    • Provides fieldwork education for occupational therapy and occupational therapy assistant students
    • Participates in continuing education for professional development to ensure practice consistent with best practice and to meet Indiana State requirements
    • Participates in multidisciplinary meetings at schools or with early intervention teams to review evaluation results, integrate findings with other disciplines, offer recommendations, and develop evidenced based intervention plans including IEP and IFSP development
    • Ability to attend and conduct in-service training programs and provide consultation to agency personnel
  • Record Keeping
    • Submit billing within appropriate time frame
    • Ensure the quantity and type of services provided is documented by the IFSP meetings and are reflected in the IFSP
    • Complete progress notes, regular evaluations and quarterly reports to be submitted to families, Service Coordinator and SPOE. Synthesizes evaluation results into a comprehensive written report which reflects strengths and barriers to patient participation in current expected roles and environments; and guides evidence-based intervention
    • Maintain all confidential files according to First Steps and Sycamore Services requirement
    • Manages inventory, fit, and cleanliness of therapeutic equipment and assessments, and project needs for budget planning
    • Documents occupational therapy services to ensure accountability of service provision, meet all Indiana State Practice Act, HIPPA, FERPA, third party payer and Sycamore Services requirements in accordance with professional standards and accepted best practice standards
  • Sycamore Services & Cornerstone Teams

    • Work with a variety of individuals in different settings
    • Perform other duties as needed.
