Columbus Health and Rehab has an excellent opportunity for an experienced RN MDS Coordinator. The ideal candidate will be a strong team leader, will have experience with MDS 3.0 and PDPM. This position requires the ability to work well with residents, families and staff.

Requirements:

Current Wisconsin RN License
1-2 years of experience as an MDS Coordinator is desired.

Point Click Care experience is a plus.
Excellent organization skills.
