Summary:
Under the supervision of the Community Resource Center (CRC) Coordinator, the Case Manager will service all clients in need of basic needs such as, but not limited to; food, shelter, clothing, medical insurance, translation, advocacy and/or other services as needed.
Essential Functions:
- Provides direct services to clients with emergency services, such as referrals for food & housing, SSI papers, translations, job information and other community resources referrals.
- Maintains accurate and updated clients' files with mandated documentation as per contracts and agency policies and guidelines.
- Provides instruction in job seeking and participates in the development of job opportunities for clients.
- Monitors progress toward accomplishments of client's goals and objectives.
- Monitors programs goals to insure compliance with contract provisions.
- Enter and maintain accurate client's information into COMET data base and/or other database as assigned.
- Participates in the development of proposals, program planning and new initiatives.
- Prepares statistical and narrative reports as required by funders and the agency's established policies and procedures.
- Participates in community activities as needed and committees especially when issues affect the Hispanic family.
- Attends all staff meetings and events as assigned.
- Any other duties as assigned by supervisor or delegates.
Education & Experience:
- Associates Degree in Human Service. Work or related field. FDC Preferred.
- Three years' experience in the field of Human Services.
- At least two years of experience working with low-income population.
- Bilingual in English and Spanish required.
