Licensed Clinical Social Worker - LCSW - Behaviorist
Harney District Hospital of Burns, OR is looking to hire a full-time LCSW to work with HDH primary care providers to assist patients with behavioral needs and support. This position earns a competitive salary of $29 to $39 per hour. A DAY IN THE LIFE OF OUR LICENSED CLINICAL SOCIAL WORKER
You will provide an active presence to the HDH Family Care clinical teams and be available to accept warm-hand-off's and provide assessment and brief treatment interventions to patients referred by the medical providers. You will facilitate referrals and coordination to other services (e.g. substance abuse treatment, tele-psychiatry, or community resources) as needed or as directed by primary care provider. You will develop and maintain professional relationships and act as a liaison with the local mental health agency to enhance coordination of patients being referred for services at their facility. Your work will help patients effectively resolve individual barriers through follow-up, advocacy and collaboration with staff and other community service providers.
It will be necessary for you to maintain accurate and timely records of activities, case management notes, and services provided to each client. You will be part of a primary care team managing patients with chronic emotional or health problems. With that team you will teach patients, families and staff about care, prevention and treatment enhancement techniques.

QUALIFICATIONS FOR OUR LICENSED CLINICAL SOCIAL WORKER
- Master's degree in clinical social work or a related field
- Licensed Clinical Social Worker - LCSW
- Knowledge about behavioral clinical social work principles
