A DAY IN THE LIFE OF A CASE MANAGER
As a Case Manager, you provide direct assistance and case management to survivors of domestic violence that reside at our shelter. You work directly with survivors to determine areas of need, set goals, and develop a service plan. Additionally, you perform lethality assessments and crisis counseling. You provide advocacy and general case management assistance related to legal, financial, medical, mental health, educational, employment, housing, and other social services for shelter survivors and their children. Coordinating with children's services, you ensure that the children's issues are always addressed along with their parents' issues.
You facilitate linkages with and make referrals to community service providers to accomplish service plan goals. In order to assess the progress toward completing the service plan goals and identify areas requiring further assistance, you regularly conduct follow-up meetings with survivors. You are conscientious about keeping up-to-date and accurate documentation from which you compile statistical data in order to maintain compliance with established funding sources. Your job is highly collaborative, as you work closely with the case management team, community services, various advocates, and IFP attorneys to ensure effective coordination of services. You get great satisfaction out of helping families in our community thrive!
