Public Health Nurse II

Performs routine (journey-level) public health nursing work related to directly observed therapy (DOT) or Directly Observed Prevention Therapy (DOPT) in the DSHS regions to control TB. Work involves providing for the assessment, care, and treatment of TB patients, including observing the patient take medications as prescribed, documenting medication compliance, assessing the patient for adverse reactions, and assisting with patient monitoring activities as assigned. Works under moderate supervision, with limited latitude for the use of initiative and independent judgment.
Under the standing delegation orders of the regional medical director and the supervision of a DSHS nursing case manager (NCM), performs directly observed therapy or Directly Observed Prevention Therapy in the DSHS regions to control TB. The DSHS region nursing supervisor or NCM will provide written instructions to the treating physician. In accordance with these instructions, the treating physician will prescribe the frequency that DOT is to be performed. This position works with the NCM or designee to determine the needed dose to be given to the patient.Provides DOT/DOPT services by directly observing the patient ingest a dose of medications prescribed by a physician for the treatment or prevention of TB infection. Delivers prescribed medications to patients at prearranged times and locations (travel required) as agreed upon with the patient using a method that ensures and maintains patient confidentiality and medication compliance. May deliver prescribed oral medication or if necessary injectables to the patient using a method that ensures confidentiality and medication compliance. Verifies that the medication(s) in the dose packet match what is listed on the dose packet label (visual check for correct pills or capsules and correct number of pills or capsules in packet). Verifies that the patient is the correct patient listed on the medication orders. If this is the Individual or entity's first visit to the patient, asks the patient to state his name or otherwise confirm identification. If the patient is a child, asks the parent or guardian to identify the child through similar processes. Verifies that the dose packet indicates the patient's correct name
Documents compliance or non-compliance with ingestion of the DOT/DOPT dose on the appropriate form provided by the region. Ensures patient compliance with TB infection treatment regimens. Notifies immediately the NCM 100% of the time if a patient does not show for scheduled DOT dose. Notifies the NCM of all changes regarding client care and medication. Completes and ensures completion of required program documentation.
Abides by all DSHS Standing Delegation Orders, policies and procedures relating to the delivery of DOT/DOPT services and within assigned responsibilities. Completes all required training associated with essential job functions and other assigned duties. Maintains strict confidentiality of patient information as provided by law and agency confidentiality procedures and policies. Keeps required licenses and credentials current.
Performs related work as assigned.
Experience and Education
Experience in nursing work. Graduation from an accredited four-year college or university with major coursework in nursing or from an accredited nursing program is generally preferred. Experience and education may be substituted for one another.
Registration, Certification, or Licensure
Must be licensed as a registered nurse by the State of Texas or a state that recognizes reciprocity through the Nurse Licensure Compact.
Knowledge, Skills, and Abilities
Knowledge of nursing techniques and procedures; of public health laws and regulations; of community health and nursing care principles, practices, and procedures; and of quality assurance principles and procedures.
Skill in using reasoning to identify problems, in considering appropriate resolution of situations, and in the use of a computer and applicable software.
Ability to conduct community assessments, to provide patient care, to prepare and maintain records, and to communicate effectively.
